With the approval of National President Brother John W. Woodhous (1923 -1925), the Delta Sigma Chapter was chartered on Sunday, December 10, 1923 in Winston-Salem, North Carolina by five noble men, whose mission was to perpetuate the high ideals of Sigma through brotherhood, scholarship and service.  These individuals were: Dr. William H. Bruce, Dr. Ashley L. Cromwell, Dr. Rufus S. Hairston, Dr. Jon R. Henry, and Dr. Cicero Neely. 
 
After completion of such prestigious schools as Howard University School of Dentistry, Meharry Medical College, and Leonard Medical School of Shaw University, these distinguished gentlemen started their practices in the Columbia Heights section of the city.  This section was in and around Winston-Salem Teachers’ College, which later became Winston-Salem State University.  Their prestige as doctors, as well as Men of Sigma quickly won them influence and status.

Delta Sigma has the distinction of having three members inducted into the Distinguished Service Chapter (DSC).  This is the highest honor a brother in Phi Beta Sigma can receive nationally.  They are recognized as Honorable brothers.  
  • Honorable Brother John F. Lewis (DSC#39)- Inducted in 1952 at the Richmond, Virginia Conclave
  • Honorable Brother Marshall B. Bass (DSC#97)- Inducted in 1985 at the Louisville, Kentucky Conclave
  • Honorable Brother Darryl A. Prince, Sr. (DSC#177)- Inducted in 2015 at the Little Rock, Arkansas Conclave
Delta Sigma has the distinction of having four members inducted into the James T. Floyd (JTF) Hall of Fame Chapter.  This is the highest honor a brother in Phi Beta Sigma can receive in the Southeastern Region.  They are recognized as Notable Brothers. The James T. Floyd Hall of Fame was established at the 2003 Regional Leadership Conference in Charlotte, NC to honor brothers who have made significant contributions to the Southeastern Region and their respective communities. This was an initiative of the Honorable Bro. Jimmy Hammock, Regional Director, and 33rd International President. Charter members class were members of the Distinguished Service Chapter of the Southeastern Region. The first class of inductees was installed at the 2004 Southeastern Regional Conference in Chattanooga, TN.

Phi Beta Sigma has convened in Winston-Salem for two International Conclave Conventions.  Delta Sigma served as host chapter for both occurrences.
  • 24th Anniversary Conclave in 1938 at Carolina Hall (Campus of Winston-Salem Teachers’ College).  The 1938 conclave was in a sense preliminary to the 1939 Conclave that would mark the silver anniversary to the Fraternity.  During the Omega Service, Honorable Brother James Weldon Johnson was committed into the Omega Chapter.  Also during this Conclave, Dr. Ivorite L. Scruggs received his Distinguished Service Key. 
  • 56th Anniversary Conclave in 1970 at the Robert E. Lee Hotel (Downtown Winston-Salem).  Thirty-two years after hosting their first conclave, the Conclave returned to Winston-Salem and the Southeastern Region.  At the 1970 Conclave, specific steps were taken to initiate a campaign for contribution of funds to erect and equip “A Sigma Shrine and permanent National Offices’ Headquarters.”
